Course Outline:
Develop an understanding of the principles, statutory guidance and best practice for supporting young people as they transition into adulthood, including those with SEND and Education, Health and Care Plans (EHCPs)
Gain practical strategies to promote independence, resilience, self-advocacy and life skills that prepare learners for successful adult lives
Understand how to support learners in exploring education, employment and training pathways through effective careers education, guidance and employability support
Build confidence in working collaboratively with families, employers and multi-agency professionals to deliver person-centred transition planning and coordinated support
Learn how to implement inclusive, learner-centred approaches that improve outcomes and enable young people to achieve greater independence, wellbeing and participation in their communities
